Important information for our NEW PATIENTS:

- Please DO NOT self schedule. You are not able to schedule a longer appointment as new patient. If self scheduled, we will have to cancel your appointment.

-We are scheduling new patients only after reviewing labs/referrals/chart information. Please either send this to us directly, or ask your medical team to send it to us.

- You are welcome to use the link above to create your chart. Once done, please either send us a portal message or call us. We will then help you with scheduling your new patient appointment.
