As of 10/1/25 telehealth services are no longer covered for you. Please call our office or send us a portal message and we will schedule you for an in person appointment.
This does not apply to patients with Medicare Advantage plans.

Please note that you can only schedule a telehealth appointment (video visit) through our link at this time. We are scheduling in-person appointments on Fridays only. For scheduling an in-person appointment, please contact our office.

- We are scheduling new patients only after reviewing labs/referrals/chart information. Please either send this to us directly, or ask your medical team to send it to us.
- You are welcome to use the link above to create your chart. Once done, please either send us a portal message or call us. We will then help you with scheduling your new patient appointment.
