Instructions on Connecting Diabetes Device to Our Clinic:

Please find your pump/sensor brand below and follow the instructions to assure we are connected to your device and can see the updated data. We recommend doing this 3 days prior to your appointment (and no later than 1 day before) to assure your appointment goes smoothly and doesn’t get delayed/cut short due to technology errors.

LIBRE Continuous Glucose Monitors

Here are steps how to get linked using a mobile app:

  1. Open your Libre mobile app.

  2. Click the three lines on the top to open the Settings Menu.

  3. Select Connected Apps from the Menu

  4. In the LibreView section, click 'Connect'.

  5. Type in our room code: seattleendo (not case sensitive)

  6. Click on Accept on the bottom of the screen.

  7. You should be shared.

 

Here are steps how to get linked through the LIbreView website using a web browser:

  1. When logged in to your LibreView account click at the top right corner.

  2. Then open up 'Account Settings'.

  3. Click the tab 'My practices'.

  4. In the 'Link to a practice' box type in the code “ seattleendo “ then click 'Add'.

  5. This will link your Libre account to our clinic's LibreView account.

 

Please let us know once you are linked and we can check on our end to assure you are connected with us.

DEXCOM Continuous Glucose Monitors

If you use Dexcom app, you can authorize sharing with our clinic from your phone follwing these instructions: 

  • Download the Clarity App. 

  • Tap on Profile.

  • Tap on Manage Data Sharing.

  • Tap Continue.

  • Enter the following Clinic Code: " seattleendo "

If you use a Dexcom G7 reciever, go to https://connect.dexcom.com on your home computer and follow the onscreen instructions to create an account. Once your account is created:

  • Go to Settings.

  • Scroll down to Data Sharing with Clinics.

  • Enter the following Clinic Code: " seattleendo "

TANDEM Pumps

Step 1: Sharing data with clinic (please make sure step 1 is done and then move to step 2 to connect your data to our clinic)

  • Option 1: mobile app: If you use your mobile app (Tconnect) and keep the app open at all times, the data will go to the clinic.

  • Option 2: upload to computer: link on how to upload it. You will need your Tandem charging cable. The process is web-based (no software) and needs to be done via a laptop or desk – it’s really fast (like under 5 minutes).   • Follow these steps to upload pump data to your Tandem Source account (depending on your operating system, your interface may look slightly different):

  1. Log in at source.tandemdiabetes.com

  2. Plug your pump into your computer using the Tandem USB cable

  3. In the menu, click “Upload Pump”, then “Start Upload”

  4. If you see a box saying “Tandem Virtual COM Port”, click on it and then click Connect

  5. “Your reports are ready to view” is displayed after the data has successfully been uploaded.

Step 2: Connecting patient account to the clinic

  1. Log into your Tandem Source account (source.tandemdiabetes.com)

  2. Look at top right of screen, find the blue circle with your initials and click to get to User Profile Navigate to the User Profile

  3. Click Account Settings

  4. Click Share Reports 

  5. Click Add Clinic to search for clinic

  6. Select Country, State, and search for the clinic by name or address (Search by Washington first and then Seattle Endocrine clinic)

  7. Select clinic and click Start Sharing

  8. The customer service team is great at troubleshooting issues with your browser and/or navigating the website. If you need additional help, please call 877-801-6901.

OMNIPOD5 Pumps

Omnipod 5 connection to Glooko needs to be setup through the PodderCentral account:

·        Must use desktop or lap. Will not work through phone or tablet.

·        Connect to clinic code: seattleendo

MEDTRONIC Pumps

Here is what you will need:

-pump

-contour meter OR blue flashstick OR the 770G mobile app

-if you do not have the contour meter, flashstick and do not use the app call 1-800-646-4633 to have one shipped to you

-computer with USB (unless you are using the mobile app)

 

500 and 600 series pump uploads:

-visit Carelink.minimed.com (works best on chrome or explorer)

-sign in with username and password if you have an account OR create an account if you do not have one/have forgotten the login

              - You will share your username and password with your healthcare team.

-follow the attached document to install the CareLink uploader (CareLink Personal Install-Guide)

-follow the attached document to upload your pump (CareLink Personal Upload-Guide)

-If you have any issues with the computer/software, please call Medtronic software team at 1-800-646-4633 ext 21003.

-once the upload is complete, please send us a portal message with your username and password to view the upload.

 

700 series pump uploads (not using the mobile app)

If you have the mobile app:

-make sure you are logged in

-click on your menu tab on the top left of your screen

-select “sync to CareLink”.

-select “upload now”.

-Send us a portal message with your username and password for the app.
